Does the request form collect medical information?
No. It asks for a name, a phone number, the reason for the visit in general terms and preferred days. Anything clinical belongs in your own practice system, not a website form.
Can you list the insurance plans we accept?
Yes, if you give us the list and keep us told when it changes. Many practices prefer a line saying you will check a plan before the visit, which never goes out of date.
